New Patient Appointments and Forms

Please bring the following to your first appointment:

• Your insurance card
• Physician referral forms, if required by insurance
• A list of current prescription or over-the-counter medications you are taking, including dose and frequency
• Previous neurological evaluations and / or records
• Any MRI films you may have
• New patient registration form (PDF format)*
• New patient encounter form (PDF format)*

You may print and complete the PDF forms above prior to your visit and bring the completed forms with you to your appointment.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may require you to pay a co-payment at the time of your appointment. Questions regarding your coverage and benefits should be directed to your employer or insurance company.
